Commissioning Appointment Request

The Township offers Commissioner of Oaths services for certain types of documents. Commissioning Services are being offered in person and remotely via Zoom.

Township staff must witness documents being signed after administering a Declaration, Oath or Affirmation. Do not sign the document in advance of your appointment. If the document is signed prior to the appointment, the Commissioner will not be able to complete the document.

Requests for commissioning appointments must be made at least 1 business day prior to the desired appointment day.

Please note this form is only a request for a Commissioning appointment.
Your appointment time will be confirmed by Township staff.

Virtual Commissioning Requirements

Remote commissioning is video and audio recorded as required as part of ‘record’ in accordance with the Commissioner for Taking Affidavits Act. The video is record keeping purposed only and is not used or shared for an

Below is a list of requirements for remote commissioning to be successful:

  • Reliable access to internet for the use of audio and video via a Zoom meeting
  • A valid email address where documents can be sent for signing during the meeting (each signee is required to provide their own email address)
  • A copy of a valid government issued photo ID sent 24 hours prior to the meeting time

Payment is required prior to the appointment for commissioning. Township staff will contact you regarding payment options.

Please note planning applications are exempt from commissioning fees.
